Audit Trail of User accesses and actionns

In order to complete our validation of JAMA, we would greatly appreciate that the hosted environments get the ability to generate user access logs that would include things like:

• failed login attempts to our domain (time stamp, credentials used, etc.) 
• user logins (time stamp)
• user account actions (pwd changed event, permissions changes, etc.)

I'm sure there is more I'm forgetting, but these are needed ASAP.  Thanks.
