Experience with Item Threshold in Projects?

In documentation Jama suggests keeping projects below the 25,000 item threshold for performance reasons, but can anyone speak to their direct experience with this? Have people been able to have larger projects no issue? Do people truly notice the performance issues?

Trying to do some planning on how to organize my projects before we reach that item threshold.
