Error when user attempts to sign in for the first time

Hello,

I believe I have set up this new user correctly and do not see duplicate user names in my list of users.

But when the new user attempts to sign in, he gets this message:

I struck out the username for privacy. I interpret this error as setting up a new user with a username that already exists. But when I check my existing list of users (active and inactive), I don't see this username being used twice.

Any suggestions?
